Certified Copies of Birth, Death & Marriage

Certified copies of birth, death, and marriage records may be obtained from the Office of the City Clerk in person, during regular business hours, ordered Online or by mail by submitting this form (PDF). The fee for a certified copy of a vital record is $10 each. If you have additional questions, you can contact the City Clerk's Office by phone at 617-715-8686 or by emailing City Clerk.

You can obtain a copy of birth, death and marriage record from the Commonwealth from 1926 to the present. You can also make an inquiry if you do not know the municipality of the original record you need to obtain.
